Translation
I am tortured by the fierce power of Your terrible fever weapon, which is cold yet burning. All embodied souls must suffer as long as they remain bound to material ambitions and thus averse to serving Your feet.

“My dear Lord, I beg to submit that I have been very greatly chastised by the release of Your Nārāyaṇa-jvara, which is certainly very cooling yet at the same time severely dangerous and unbearable for all of us. My dear Lord, as long as one is forgetful of Kṛṣṇa consciousness, driven by the spell of material desires and ignorant of the ultimate shelter at Your lotus feet, one who has accepted this material body becomes disturbed by the three miserable conditions of material nature. Because one does not surrender unto You, he continues to suffer perpetually.”
Purport
In the previous verse, the Śiva-jvara stated that those who live by violence will suffer similar violence at the hands of the Lord. But here he further states that those who do not surrender to the Supreme Lord are especially liable to punishment. Although the Śiva-jvara himself had acted violently up till now, since he has surrendered to the Lord and rectified himself, he hopes to receive the Lord’s mercy. In other words, he has now become the Lord’s devotee.

I cannot praise you any longer because of the weapon sent by you is both cold and burning. Someone may say, "It is fitting that you suffer since you cause suffering to others." He answers, "Those who do not surrender to your feet deserve to suffer. But I have become your devotee."

Having surrendering he states his purpose. I am suffering because of your intolerable power in the form of the jvara. It is cool for the devotees, just as the woman saw you as Cupid, but burning fiercely for me, just as the wrestlers saw you as a thunderbolt. That weapon is extreme (ulbanena). All suffering, what to speak of only the suffering from the jvara, will continue as long as people do not surrender to your feet. They do not surrender because they are completely bound by aspirations for sons, wealth and other persons. Or, those bound by thirst for your mercy serve you.
Purport (Sanatana Goswami)
First protect me from the heat. “But you are the great Lord? Why would you suffer?” It is your heat, most intolerable because the heat is ferocious, being both cold and cruel, more than mental pain or the pain of saṁsāra. Those who identify with bodies or act according to the body’s nature are qualified for suffering as long as they do not somehow even a little serve a portion of your foot.
Such persons are bound (baddhāḥ) slightly (anu) by desire for service which takes away the pain, or even just begin to serve. Now, by surrendering to you, I do not deserve any pain, what to speak of the pain created by the jvāra.
